For chronic conditions and complex care needs, you could be eligible for a Medicare Care Plan from your GP.

To qualify for a Care Plan, you must have a condition that has been present for 6 months or longer. We see clients with hypertension/high blood pressure, rheumatoid arthritis, osteo arthritis, fibromyalgia, asthma, diabetes, hyperlipidemia, cancer, osteoarthrits – many different conditions qualify.

If you’re eligible, you can receive subsidised allied health appointments through Medicare to see our Clinician. 

We will need a copy of your GPs referral prior to booking your initial consultation with your selected Allied Health professional. Simply reach out to us to get the ball rolling!
